Complex Case Management are hiring a Full Time support worker to help provide support to our young person in Poulton Le Flyde We’re looking for people with a genuine passion for caregiving, and who have a proven track record of providing excellent service. Experience and background in Rehabilitation and Brain injury is essential.
You must be an enthusiastic, motivated, assertive & friendly Rehabilitation Support Worker, working within the home and in the community.
Applicants must be:
Minimum age of 25 years – Due to insurance on WAV Vehicle.
Full UK driving license holder.
Be able to Swim – Hydrotherapy sessions with Client.
Responsibilities:
Provide care for a young Person within their own home and in the wider community
Maintain a safe and clean environment in the client’s home.
Follow instructions from the Multi-disciplinary team.
Monitor and report on changes in the client’s health.
Check in with the client’s family members to help ease their stress.
Handle any emergency situations involving the client.
Attend Hydrotherapy sessions.
Support with personal care needs.
Maintaining the young person’s mobility
Maintaining the young person’s general health
Maintaining the young person’s personal care to include:
Washing /Bathing
Dressing
Teeth cleaning
Nail care
Continence care
Maintaining the young person’s nutrition
Maintaining the young person’s physical management
Supporting the young person’s therapeutic engagement
Supporting the young person and engaging in Hydrotherapy
Providing appropriate leisure activities for the young person
Contribute to all household duties – including cleaning of equipment
Use of Mobile Hoist
Driving the young person’s adapted vehicle, must be over 25years old
Maintaining the young person’s safety in all environments
Must also love dogs as the young person has a large dog.
General:
Promote, monitor and maintain health, safety and security in the workplace.
Promote effective communication and relationships.
Contribute to the protection of individuals from abuse.
Establish and maintain a trusting, therapeutic, working relationship
Be responsible for the young person’s personal welfare
Travel to hospitals or elsewhere for therapeutic care
Liaise with other agencies as required
Complete daily records as advised including; time sheets, daily record sheets, medication sheets, petty cash sheets and critical incidents.
Attend Team Meetings/review meetings with the employer and other persons as nominated by the employer
Attend for regular Formal Supervisions and Annual Appraisal
Attend training sessions as arranged
Successful candidates will be working within the clients own home . You will receive a full induction programme and continued training from qualified Therapists, Psychologists and Nurses.
