We are seeking an Experienced Rehabilitation Case Manager to provide extra support and services to Clients with an Acquired Brain Injury. You will have the opportunity to make a significant difference in their lives and empower them to achieve their rehabilitation goals. You will be responsible for developing and monitoring rehabilitation programmes, mentoring support staff, and representing the interests of the client. Responsibilities:
- Administer rehabilitative programs to Clients
- Provide leadership to staff in the provision of services, including data collection and analysis, quality improvement, fiscal oversight, and resource allocation.
- Conduct initial needs assessments and develop plans of care.
- Communicate with physicians, agencies and others to coordinate discharge planning and patient follow-up for inpatient rehabilitation services.
- Provide clinical supervision for employees
- Participate in professional education activities to ensure employees are competent to carry out responsibilities.
- Manage fiscal resources to ensure efficient operation of programs.
- Monitor the effectiveness of performance measures developed by staff.

What is Case Management?
Case managers are healthcare professionals (e.g Registered Nurses, Social Workers, Physiotherapists, Occupational Therapists) who help provide an array of services to assist individuals and families cope with complicated health or medical situations mainly spinal and acquired brain injury in the most effective way possible, thereby achieving a better quality of life. They help people to identify their goals, needs, and resources. From that assessment, the case manager and the client together formulate a plan to meet those goals.
The case manager helps clients to find resources and facilitates connection with services and rehabilitation programs. Sometimes she or he advocates on behalf of a client to obtain needed services. The case manager also maintains communication with the client, professionals involved in their rehabilitation, family and the care team to evaluate whether the plan is effective in meeting the client’s goals.
About You
- You must have a registered clinical qualification (either HCPC/NMC) OT,SALT, Physio. Social Worker, Registered Nurse, etc.
- A minimum of 2 years post qualification experience essential in the field of acquired brain injury.
- Evidence of community based work would be desirable
- A full clean driving license and access to a car is essential
- Full support, training and mentoring will be provided to the successful applicant
