Preston PR3 – Rehabilitation Support Worker

Job description

17 year old female with cerebral palsy, looking for enthusiastic, motivated and flexible workers to join my team of rehabilitation support workers, working within my home and in the community.

I love to go out and do activities such as, shopping, cinema and quiz nights; generally anything that gets me out and about and experiencing new things. I enjoy spending time in school and socialising with my friends.

I enjoy indoor activities too, like spending time playing games with my younger brothers, pamper sessions, listening to music.

My days can be quite full with therapy activities, such as hydrotherapy, standing in my movement harness and passive stretches.

I use alternative communication techniques, but do like to socialise so need my support workers to have excellent communication and interpersonal skills.

I do need help with my medical needs, such as PEG Feed, medication administration and personal care, so previous health care experience is essential.

The successful applicant will be fully trained in all aspects of my clinical care needs. You will be well supported and supervised by the management team at Complex Case Management. They provide bespoke rehabilitation and personal care services to clients in the community, throughout the North West and are recruiting for this post on behalf of my finance deputy.

The post requires that you:

· Are female (Due to personal care)

· Non Smoker (my request)

· PEG Feeding experience (Preferred)

· work independently to a great extent

· work and conduct yourself in an acceptable manner

· can cover weekdays; weekends and night duty (waking nights)

· can work flexibly, along with the ability to work additional hours as required

The post is part time/ full time, permanent- Must be willing to work Nights and Weekends

4 positions available:

Person 1- 42 hours per week

Shifts- every weekend 12 hour days and wake nights plus weekday hours

Person 2- 28 hours per week

Alternate weekends ( 12 hour day shifts), Weekday waking night shifts (12 Hours) plus afternoon shifts (4 hours)

Person 3- 20 hours per week

Weekend night shifts (12 hours) alternate, weekday shifts 4 hours per day

Person 4- 32 hours per week

Every weekend (12 hour days week 1, 12 hour nights week 2),weekday shifts 4 hours per day

Rate of pay:

During 6 month probation period, weekdays paid at £13.50 per hour and weekends paid at £14.50 per hour.

Once probation has been passed, the rate will go up, weekdays paid at £15.00 per hour and weekends paid at £16.00 per hour.

Fully clean UK driving licence required.

An enhanced DBS will be required for this post.

Key Tasks and Responsibilities:

Specific:

· Maintaining the young person’s mobility

· Maintaining the young person’s general health

· Maintaining the young person’s personal care to include:

  • Washing /Bathing
  • Dressing
  • Teeth cleaning
  • Nail care
  • Continence care

· Maintaining the young person’s sleep pattern

· Maintaining the young person’s nutrition

· Maintaining the young person’s physical management

· Supporting the young person’s therapeutic engagement

· Providing appropriate leisure activities for the young person

· Contribute to all household duties – including cleaning of equipment

· Driving the young person’s adapted vehicle

· Supporting the young person on family holidays

· Maintaining the young person’s safety in all environments

General:

· Promote, monitor and maintain health, safety and security in the workplace.

· Promote effective communication and relationships.

· Contribute to the protection of individuals from abuse.

· Establish and maintain a trusting, therapeutic, working relationship

· Be responsible for the young person’s personal welfare

· Travel to hospitals or elsewhere for therapeutic care

· Liaise with other agencies as required

· Complete daily records as advised including; time sheets, daily record sheets, medication sheets, petty cash sheets and critical incidents.

· Attend Team Meetings/review meetings with the CCM and other persons as nominated by the employer

· Attend for regular Formal Supervisions and Annual Appraisal

· Attend training sessions as arranged
