Complex Case Management is a leading provider of tailored, person-centred care in the community across the Northwest. We are currently seeking a compassionate and dedicated Rehabilitation Support Worker to join our team in supporting a 45-year-old male client with an Acquired Brain Injury (ABI). The role will initially be 36 hour per week over 3 days, offering flexible hours as you work alongside his current rehabilitation team until the client moves into his own home. Following this transition, the role will become 42 hours per week including sleep-in shifts on a rolling rota. .

Key Responsibilities:

  • Assist the client in achieving rehabilitation goals, including support with physical therapy and exercise routines.
  • Facilitate attendance at various therapy appointments (e.g., physiotherapy).
  • Help with daily tasks such as meal preparation, graded cooking, leisure activities (e.g., Boxing), and maintaining a structured routine.
  • Promote the client’s health and well-being by assisting with nutrition, fatigue management, and overall wellness.
  • Support the client in maintaining safety and independence in different environments, including during outings and activities.
  • Contribute to household duties to ensure a clean, comfortable, and safe living space.
  • General Duties:
  • Ensure health, safety, and security at all times within the workplace.
  • Build positive relationships and foster effective communication with the client, their family, and the multidisciplinary team (MDT).
  • Participate in safeguarding measures to ensure the client is protected from abuse.
  • Maintain accurate records of care, including daily logs, medication, and incident reports.
  • Attend team meetings, case reviews, and regular supervisions with the Complex Case Management team and MDT.
  • Take part in ongoing training to continuously improve and enhance your skills.

Essential Qualifications and Skills:

  • Experience in care, particularly with clients with Acquired Brain Injury (ABI), is essential.
  • A compassionate, proactive, and motivated individual who can work both independently and as part of a team.
  • A minimum of 2 years’ experience in care work, ideally in a rehabilitation or ABI setting.
  • Excellent communication and interpersonal skills.
  • A valid, clean full UK driving licence with business insurance.
  • Enhanced DBS check required.

Working Hours:

Initially 36 hours per week, transitioning to 42 hours per week after the client moves to their own home.

Flexibility is essential to cover weekdays, weekends, and bank holidays as needed.

Hours may vary depending on the client’s needs.
