Team Leader Rehabilitation Support Worker – Burnley

Position Available: Rehabilitation Team Leader – Support Worker

Job Type: Full-time

Salary: £15.00 per hour – Sleep shifts paid at minimum wage.

*Key Responsibilities:*

Team Leader Job Specification:

To be responsible for: –

•⁠ ⁠Informing Case Manager/Case Manager Assistant of any risks identified and any problems, etc.

•⁠ ⁠Ensure accurate completion of all carer timesheets

•⁠ ⁠Collate timesheets on a monthly basis & send to CCM HR Manager

•⁠ ⁠Taking calls from staff for sickness, covering Sickness following the on-Call Staffing procedure. Informing Case Management Assistant and/or Case Manager of any sickness.

•⁠ ⁠Undertake preparing rota under the supervision of Case Manager Assistant at least 6 weeks in advance.

•⁠ ⁠Ensure that the shifts are covered re: carer absences, etc.

•⁠ ⁠Monitor the delivery of the care plan in line with written guidelines.

•⁠ ⁠Ensure completion of the care plan record sheet at the end of each shift.

•⁠ ⁠Ensure Daily interventions are fully completed before staff finish shifts.

•⁠ ⁠Collation of Petty cash/finances

•⁠ ⁠Ensure all Health & Safety guidelines are observed, and Policies adhered to

•⁠ ⁠Inform case manager of any breach of Health and Safety guidelines

•⁠ ⁠Coordinate designated tasks – equipment, environment, vehicle, and supplies.

•⁠ ⁠Contribute to the staff appraisal process.

•⁠ ⁠Coordinate training, shadow shifts and induction of new staff

•⁠ ⁠Liaise with case manager and Multi-Disciplinary team re: appointments.

•⁠ ⁠Ensure PPE Policies and Procedure and adhered to.

•⁠ ⁠Ensure PPE Stock levels are fully stocked, Ordering PPE when needed.

•⁠ ⁠Completion of weekly medication audits

•⁠ ⁠Completion of weekly first aid audits

Support Worker Key Tasks and Responsibilities:

•⁠ ⁠Promote, monitor and maintain health, safety and security in the workplace.

•⁠ ⁠Promote effective communication and relationships.

•⁠ ⁠Contribute to the protection of individuals from abuse.

•⁠ ⁠Enable clients to maintain their personal hygiene and appearance.

•⁠ ⁠To complete domestic tasks, i.e. cooking, cleaning, laundry.

•⁠ ⁠Enable clients to participate in recreation and leisure activities.

•⁠ ⁠To establish and maintain a trusting, therapeutic, working relationship

•⁠ ⁠To be responsible for the client’s personal welfare

•⁠ ⁠To supervise activities in line with the care plan guidelines

•⁠ ⁠To travel to hospitals or elsewhere for therapeutic care

•⁠ ⁠To travel for social or other activities

•⁠ ⁠To liaise with other agencies

•⁠ ⁠To complete daily records as advised including; time sheets, care plan summary and critical incidents, petty cash.

•⁠ ⁠Attend review meetings with the employer and other persons as nominated by the employer

•⁠ ⁠To attend training sessions as arranged

*Job Requirements:*

•⁠ ⁠Excellent communication and interpersonal skills

•⁠ ⁠Previous healthcare experience preferred

•⁠ ⁠Ability to work independently and with a team

•⁠ ⁠Willingness to undergo training

•⁠ ⁠Commitment to following health and safety guidelines
